Appendix T: SAMPLE Residency Interview Agreement

The State University of New York College of Optometry (the “College”) has shifted its 2021-2022 residency interview process from an in-person one to a virtual one. In order to ensure that the process is both confidential and fair for all applicants, the College requires that no applicant shall record any portion of their residency interview. The prohibition on recording includes any part of a residency interview conducted on any virtual platform, including but not limited to Zoom, WebEx, and Skype. The prohibition extends to video recordings, audio recordings, screenshots, and still photos. By signing below, the applicant agrees that by accepting an interview with the College, the applicant will not make, or cause to be made, any video, audio, or photographic recording of any kind of any portion of their residency interview. The applicant acknowledges that they are waiving a legal right they may otherwise have had by virtue of the law of the state in which they are located, and acknowledge that they are doing so knowingly and for the express purpose of securing an interview with for the College’s residency program.
