Initial Request
Bi-ennial Renewal **Requirements must be met within the credentialing cycle** Satisfactory review by the Quality Assessment and Improvement Team of the medical records of the applic ant’s post-operative management of three (3) patients who have undergone non cataract ophthalmic surgery within the UEC or documentation of providing similar care to three patients external to the UEC, AND Two (2) hours of COPE-approved CE in the area of surgical management of non cataract ophthalmic surgery. Satisfactory review by the Quality Assessment and Improvement Team of the medical records of the applicant’s post-operative management of five (5) patients who have undergone refractive surgery within the UEC during the two year cycle, or documentation of providing similar care to five (5) patients external to the UEC, AND Four (4) hours of COPE-approved CE in the area of post-operative management of refractive surgery. Satisfactory review by the Quality Assessment and Improvement Team of the medical records of the applicant’s performance of ocular fluorescein angiography of five (5) patients within the UEC, or documentation of providing similar care to five (5) patients external to the UEC, AND Four (4) hours of COPE-approved CE in the area of retinal vascular disease diagnosis and/or fluorescein angiography. Satisfactory review by the Quality Assessment and Improvement Team of the medical records of the applicant’s performance of electrodiagnostic testing of two (2) patients within the UEC, AND Ten (10) cases of electrodiagnostic testing completed by the applicant.
Advanced Care
Post-op Management of Non-Cataract Ophthalmic Surgery
Completion of an ACOE accredited Residency Program or Fellowship in Ocular Disease or Primary Care or documented experience in providing post operative management of patients who have undergone non-cataract ophthalmic surgery.
Perioperative Management of Refractive Surgery Patients
Completion of an ACOE accredited Residency Program or Fellowship in Ocular Disease, Primary Care, Cornea and Contact Lenses, Diplomate in Cornea, Contact Lenses and Refractive Technology, OR Documented experience in providing post-operative management of refractive surgery patients. Completion of an ACOE accredited Residency Program or Fellowship in Ocular Disease or Primary Care, OR Documented experience in performing ocular fluorescein angiography.
Ocular Fluorescein Angiography
Electrodiagnostic Services
Satisfactory completion of in house training in the use of the VERIS system (new machine) by clinicians currently privileged in the use of this system, OR Documented experience in the use of the EDI instrument (old machine).
