Active NYS Registration In accordance with New York State (NYS) Office of the Professions and the University Eye Center (UEC), it is mandatory that any licensed provider within the UEC have an active NYS registration at all times. All communications to the provider regarding upcoming expiring NYS registration renewal will be sent via email/ certified mail as specified below: Initial Notification: A notification will be sent to the provider via e-mail (to SUNY First Class account) 75 days in advance of the registration expiration date. Second Notification: If the updated registration is not received, a follow-up email (to SUNY First Class account) will be sent 45 days prior to the provider registration expiration indicating that this is a second request. Tertiary and Final Notice: If no response is received to initial and second notices 21 days before registration expiration , a certified return receipt letter from the Chief of Staff will be mailed to the provider’s home. In addition, a copy of this letter will be forwarded to the provider’s service Chief and/or primary Supervisor and credentialing department for non-compliance with producing a copy of an active NYS Registration . If a hard copy of the NYS registration is not received by the registration expiration date, the provider’s clinical privileges will be suspended by the Chief of Staff of the UEC. This will be followed by a notification of suspended privileges to the National Practitioner Data Bank. Upon suspension of privileges, the provider’s EMR access will be deactivated and the provider will not be allowed to see patients in the UEC.
