Otherwise, you will see an Evaluation Confirmation page. In a few seconds, your Welcome page will appear again. You will notice that the evaluation you just did will either be gone from your list, or it will be marked "done" (if there's another evaluation that's still due in that row). Non-Scheduled Forms The Access Non-Scheduled Forms link takes you to a page that displays all of your institution's non-scheduled forms. The most common types of non-scheduled forms are evaluations and patient logs. However, any kind of form-based data entry falls into this category. Because of their flexibility and diversity, non -scheduled forms are one of the most commonly used features of Meditrek. Forms To access a non-scheduled form, click on its name. The data entered on these forms is validated in the same manner as the data entered on scheduled evaluations. If there are any problems with your form submission, the errors will be marked (*). Otherwise, a message will be displayed letting you know that the data was saved successfully. Records To review data that you previously entered and saved, click on the records link (paper icon). This will take you to a page that allows you to search through the individual records that you saved using that form. If the form has customized reports associated with it, those reports will be available through the records link also (in which case, you would first be presented with a choice of whether to view the reports or the individual records). When the individual records are searched, each record in the search results provides a link to view the record details. Typically, there is also a link that allows you to edit the record details. If there are event-driven evaluations associated with the form (for example: log-based evaluations), there may also be an additional link to view the evaluation associated with the record.
