Policy and Procedure: Annual and Pre-Employment Health Assessments Procedure Number: A-4 Page: 1 of 2 Effective Date: 5/6/1991 Revised: 1/16/2020 Last Reviewed: 3/12/2021 ___________________________________________________________________________ Policy: It is a Department of Health requirement that the health status of each employee is examined prior to the beginning of employment and annually thereafter. This examination must be sufficient in scope to ensure that the employee is free from any physical or mental health impairment which is of potential risk to patients or which may interfere with the performance of his/her duties. This policy will also pertain to students who come into contact with patients. students, who come into contact with patients, will be required to have a health assessment which includes as assessment of physical and mental health, as well as tuberculosis (TB) status (see 2c below for acceptable criteria), within the 6 months prior to employment (see attached Annual Health Assessment form). Continued Employment 2. All University Eye Center employees, including providers, staff, residents and students, who come in contact with patients, will be required to have the following: a) Once per calendar year, or more frequent if necessary, health status reassessment to ensure freedom from any physical or mental health impairment which is of potential risk to patients, or might interfere with the employee’s performance of duties. b) Immunization history as required on the health assessment form. c) An annual assessment of TB status. This must include one of the following means of assessment: Purified protein derivative (PPD) skin test for tuberculosis (Mantoux) for employees with previously negative findings OR The QuantiFERON-TB Gold test or the TSpot test are acceptable alternatives to the Mantoux test OR Procedure: Initial Employment 1. Employees with a history of positive PPD findings and a negative initial chest x-ray shall require an annual TB symptom screening completed by a physician, with chest x-rays and/or blood work repeated only if TB symptom screening is positive. 75
