e) Therecordsof a studentwill bedisclosedwithout his/herwrittenconsent tothose federal and state governmentagenciesandofficialsonly asprovidedby law. f) The recordsof a student will bedisclosedwithout his/herwrittenconsent to anagency towhich the student has applied or fromwhichhe/she has received financial aidorwhichhasmade decisions concerningeligibility,amount,conditionsorenforcementoftermsofsuchaid. g) Therecordsof astudentwill bedisclosedwithout his/herwrittenconsent tocertain educational agenciesandinstitutionsonlyaspermittedby law. h) TheCollegereserves the right to furnish toparentsorguardiansof financiallydependent students any informationrelatingtosuchstudents’academicstatus. i) Therecordsof astudentwill bedisclosedwithout his/herwrittenconsent tocomplywitha judicial orderorsubpoena. j) Therecordsof a studentwill bedisclosedwithout his/herwrittenconsent inahealthor safety emergency. a) InstructorssubmitgradestotheRegistrarelectronicallyviathemysunyopt.eduportal. b) Supplemental grades suchascompletions for “I”gradesor repeatcourses for “F” grades must be submittedonanofficial GradeChangeForm.Gradechangesmust be signedby the instructor, department chairandthevicepresident anddeanforacademicaffairsbefore submissiontothe OfficeoftheRegistrar. 2) ConfidentialityofGrades a) Gradesof individual students, inanyidentifiableformat,maynotbepublishedorposted.This prohibition includes the listingof gradesby social securitynumber.Thisdoesnot precludethe posting of thegradedistribution for acourseby the instructor as longasgradesarenot identified bystudent b) All personswith access togradesmust not divulge individual student grades to anyoneeither verbally orinwrittenform. 3) Availability for use by faculty andCommittees onCourse&Standing. a) Atthe requestof the vicepresidentanddean for academic affairsorassociatedean for graduate studiesand research, transcripts,gradereportsandwritteninstructorcomments maybemade availableat theendof the semester facultymeetings andatmeetings of the Course&Standing Committee. b) Transcriptsofgradesatpriorinstitutionsattended, copiesof standardizedtestscoresand lettersof recommendationmay be requestedby the Course&Standing Committeeby majority voteof the committeememberspresent.Nootherdocumentsmayberequested. c) Theregistrarshallberesponsibleforthedistributionandcollectionofalldocuments, and copies thereof,whichareutilizedat facultyCourse&Standingmeetings. All such documentsandcopies mustbereturnedtotheregistrar. d) Thechairpersonat eachmeetingwhere student recordsarediscussedmust informall present about themaintenanceofconfidentialityofallstudentrecordsandoftheproceedingsofthat meeting.
