3.1 c - Expansion of Home-Bound Program

• By 2013, an additional 1,000 homebound patient visits, the majority of whom reside outside Manhattan, will be served. Number Needs to Revised – 1,000 too much – change to 200 Queens and Manhattan – 143 in 2010 Grant Dependent work with VPIA • An additional .2 FTE will be dedicated to homebound visits by 2009 ‐ 2010.
