The following must be submitted to Dr. Diane Adamczyk, Director of Residency Education, by the dates indicated below:

Evaluation of Program Supervisor (First half) Discuss with Supervisor Paper Draft

January 15, 2021

February 1, 2021

(Supervisor may set an earlier deadline) Proof of COPE Application Completed for Majors & Handout for Major Presentation (sent to CE)

February 22, 2021 Final Paper (Publishable Quality) to Office of Residency Education April 15, 2021 (with signed Supervisor’s Approval, submitted for Writer’s Workshop II/Editorial Review) ( Any revisions recommended at Writer’s Workshop II/Editorial Review will be due 1 week after Review ) Evaluation of Program Supervisor June 11, 2021 (Final)
