Examinations of Family Members
• Students may not schedule family members or friends for themselves when they are not assigned to a Service without permission. • The student must obtain a prior commitment from the service chief and from a faculty member who must supervise and sign the medical record and the encounter form. • The student must arrange for a room assignment through the clinic chief or manager of the respective service. • All patients (even family members) must be registered in the University Eye Center with their demographic and insurance information and be scheduled with an appointment. This can be done via the front desk in the clinic or by contacting the call center. Each visit to the service must be documented in the patient’s medical record. The University Eye Center is a Diagnostic and Treatment Center licensed by the New York State Department of Health. To be seen in the University Eye Center as a patient, you will need to provide your demographic and insurance information when you are registered as a patient. After your visit, the University Eye Center will bill the claim to your insurance carrier. Individuals will be responsible for any portion of the bill considered by the insurance plan to be the "patient's responsibility"(including applicable co-pays and deductibles). The University Eye Center does not provide any discounts on ophthalmic materials such as glasses, contact lenses, and hand held devices. However, there are some contact lens companies that make contact lenses available to students. If you have financial difficulty, you can apply for a fee reduction. Fee reduction applications are readily available on every clinic floor as well as the Office of Clinical Administration (Room 1030). Patient Responsibility
