If the study was conducted in the CVRC, please obtain the CVRC Director, Dr. Kristen Fry’s signature before submitting. Please submit completed form to the Associate Dean, Dr. Stewart Bloomfield (sbloomfield@sunyopt.edu) and cc the email to the Associate Operation Manager, Mr. Cyrus Vivar (cvivar@sunyopt.edu).
