Bi-annual Renewal **Requirements must be met within the credentialing cycle** Satisfactory review by the Quality Assessment and Improvement Committee of the medical records of the applicant’s post-operative management of three (3) patients who have undergone non- cataract ophthalmic surgery within the UEC or documentation of providing similar care to three patients external to the UEC, AND Two (2) hours of COPE-approved CE in the area of surgical management of non- cataract ophthalmic surgery. Satisfactory review by the Quality Assessment and Improvement Committee of the medical records of the provider’s performance of electrodiagnostic testing of two (2) patients within the UEC, AND

Advanced Care

Post-op Management of Non-Cataract Ophthalmic Surgery

Completion of an ACOE- accredited Residency Program or Fellowship in Ocular Disease or Primary Care or documented experience in providing post- operative management of patients who have undergone non-cataract ophthalmic surgery (e.g., , glaucoma, retina, plastics). Completion of an ACOE- accredited Residency Program or Fellowship in Ocular Disease with exposure to electodiagnostic patient services, OR Completion of an apprenticeship with a qualified clinician.

Electrodiagnostic Services

Ten (10) cases of electrodiagnostic testing completed by the applicant.
